Q: How is free food paid for by insurance possible?
A: Through the 1115 waiver passed by The Centers for Medicare and Medicaid Services, states have the option to provide nutritional services to individual members who qualify. Qualifications include chronic illness and recent hospitalization. Every state has slightly different qualifications, so please check details for your specific state.
Q: Who qualifies to receive free food paid for by insurance?
A: Some people who may qualify for MTMs include:
- People with chronic conditions, such as diabetes, cardiovascular disorders, HIV, cancer, or chronic lung disorders
- People with chronic or disabling mental or behavioral health disorders
- People who have been discharged from a hospital or skilled nursing facility
- People who are at high risk of hospitalization or nursing facility placement
- People with extensive care coordination needs
Q: How do I know if my patient qualifies?
A: Performance Kitchen has created a proprietary system to pre-qualify patients based on their insurance plan, location, and condition. The system is free to use and easy to set up. Try it out here.
Q: My patients require a low sodium diet. How much sodium is contained in your meals?
A: All of the meals expertly crafted by our formally trained chefs adhere to the nutritional guardrails of chronic illness. They have also created bundles that complement each disease state to ensure food is medicine and will result in a healthy outcome for your patients. These bundles include vegetarian, low-sodium, low-cholesterol, heart-friendly, etc.
Q: How many meals do my patients receive?
A: Upon enrollment and authorization by the health plan, your patients will receive 14 meals to feed them twice daily for 7-days. Providers can renew the prescription within our professional portal when the patient becomes eligible for a refill.
Q: Is there a copay for meals?
A: Medicaid and Medicare do not require a copay.
